By indicating your acceptance, you understand, agree, warrant and covenant as follows:

I hereby, for myself, my heirs, and executors, waive, release, and discharge all rights and claims that I may have or may hereafter accrue against the organizers and sponsors of the Siena’s PWS Walk 2011, including, but not limited to, the family of Siena Schoenberger, Siena’s Special Needs Trust, the sponsors, their representatives, and successors, and South Hills Park for any injury or illness, which may directly or indirectly result from my participation in Siena's PWS Walk.

Further, I hereby grant full permission to all the foregoing to use my name, photograph, videotape, motion picture, and recording by any other record of this event without compensation.

BY INDICATING YOUR ACCEPTANCE OF THIS AGREEEMENT AND WAIVER, YOU ARE AFFIRMING THAT YOU HAVE READ AND UNDERSTAND THIS AGREEMENT AND WAIVER AND FULLY UNDERSTAND ITS TERMS. YOU UNDERSTAND THAT YOU ARE GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE. YOU ACKNOWLEDGE THAT YOU ARE SIGNING THE AGREEMENT AND WAIVER FREELY AND VOLUNTARILY, AND INTEND BY YOUR ACCEPTANCE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
